Meijer Great Choices Student Film Festival 2020 High School Registration Form
Please complete a separate registration form for each 30 second Public Service Announcement (PSA) submitted; be sure to send the registration fee and talent release forms. Your PSA will become property of the Meijer Great Choices Film Festival.
January 17, 2020 - with a $5 early registration fee
February 7, 2020 - Final deadline with $10 registration fee.
Registrations must be postmarked by the dates above to be eligible for entry into the Meijer Great Choices Film Festival.
Fill out the on-line registration form, print off a copy of the registration form and send the printed from along with your registration fee ($5 per entry if postmarked by January 17, 2020; $10 per entry if postmarked by February 7, 2020), and all talent release forms (if applicable.)
Upload your PSA on-line via a Google Shared File. Ask your teacher to email email@example.com to set up a Google Shared Drive file where you or your teacher can directly upload your PSA video. You can also upload your talent release forms and proof of purchase of copywritten music and talent release forms.
Registrations and payments must be postmarked by the above registration deadlines to qualify.

You may also use a credit card to pay the registration fees. Call 616.878.6801 or 616.878.6803 Wednesday-Friday, 12-5 pm. Visa, Mastercard and Discover are accepted.
